In-Home Caregiver (CNA / PCA)
Licensed Practical Nurse (LPN)
> Key Responsibilities

Collaboration & Communication

• Maintain consistent, hands-on communication with the RN regarding client status, care coordination, and clinical decisions

Promptly report any changes in client condition or concerns that may require care plan adjustments

Work collaboratively with the interdisciplinary care team to ensure continuity of care

Supervisory Visits (Client-Focused)

• Conduct scheduled supervisory visits in accordance with state regulations (e.g., every 60 days)
• Assess client well-being, safety, satisfaction, and caregiver performance during on-site visits
• Document visit findings thoroughly and communicate concerns or recommendations to the RN

Care Plan Support

• Provide nursing services in accordance with RN-initiated care plans
• Recommend care plan updates as needed; ensure RN approval prior to any changes
• Support wound care management and basic clinical interventions within LPN scope of practice

Oversight of Personal Care Services

• Monitor and track personal care tasks using Asana or other care coordination platforms
• Review daily caregiver documentation for accuracy, completeness, and compliance
• Identify and follow up on documentation discrepancies or red flags in coordination with the RN

Client-Centered Clinical Care

• Provide skilled nursing support to medically fragile clients
• Administer medications, monitor vital signs, and perform wound care as required
• Ensure all services comply with agency policies, procedures, and state regulations

> Qualifications

Active LPN license in the state of practice
Minimum 1 year of experience in home health, community health, or clinical nursing
Strong assessment, communication, and documentation skills
Experience with digital care coordination platforms (e.g., Asana) preferred
Ability to work independently in the field while collaborating closely with an interdisciplinary team
